Why should i fill out your survey?
We try to provide a service to cater for all out patients, unfortunately we sometime miss something or need to change things. By you completing our 5 minute survey we can see if we are doing a good job and where we need to improve. Based on the results of this survey we made decide to include a new service, or improve an existing service which may help all our patients.

Is this survey private as i dont want to give my name?
Yes. All surveys are private and confidential and can be completed anonymously, if you dont want to give your name then simply dont add it to the form.

Do i have to answer all the questions?
Its would be better if you could as it will help with our findings in general however if you feel that you are uncomfortable answering a perticular question simply miss it out.

Is this survey for the NHS?
No. This survey is for the Birley Health Centre only and will not be shared with the NHS or any other third party.

Simply contact us and we will do our best to help answer your questions or ask at reception when you next visit.
