When ordering a repeat prescription please give the following information:
Name;
Date of Birth;
Medicines required;
Where you wish to collect your prescription, e.g. Peak Pharmacy (next door), Lloyds Pharmacy (Birley Moor Road or Crystal Peaks), or at the surgery.

How to order a repeat prescription:

PHONE: 264 0066 at any time and leave a message on the answering machine.

PHONE: 264 0066 between 10:30am. and 12:00 noon Monday to Friday and speak to a Receptionist. Please note the repeat prescription telephone line is exceptionally busy on Monday and Tuesday mornings; if at all possible please ring Wednesday, Thursday or Friday morning, when the telephone line is usually a lot quieter.

FAX: 264 5814 and leave all relevant details - as above.

ONLINE: click here to be directed to the Sheffield NHS prescriptions service. You will need to register your details the first time you use the service.

CALL IN: Bring your prescription request into the surgery at any time between 8:30am and 6:00pm.

POST: If you want your prescription to be posted back to you, remember to enclose a stamped addressed envelope, as we will be unable to do this otherwise.
