When ordering a repeat prescription please give the following information:
Name;
Date of Birth;
Medicines required;
Where you wish to collect your prescription, e.g. Peak Pharmacy (next door), Lloyds Pharmacy (Birley Moor Road or Crystal Peaks), or at the surgery.

ONLINE: click here to be directed to the Sheffield NHS prescriptions service. You will need to register your details the first time you use the service.

THE ON-LINE PRESCRIPTION SERVICE WILL BE UNAVAILABLE BETWEEN THE FOLLOWING DATES 28/05/10 - 14/06/10 AS WE ARE MOVING TO A NEW CLINICAL SYSTEM. WHEN BACK ON-LINE YOU WILL STILL BE ABLE TO USE YOUR USER NAME AND PASSWORD AS BEFORE. IN ORDER TO GET READY FOR THE NEW SYSTEM WE WILL START TO ISSUE TWO MONTHS WORTH OF PRESCRIPTIONS TO SEE US THROUGH THE INITIAL GO LIVE PERIOD.

CALL IN: Bring your prescription request into the surgery at any time between 8:30am and 6:00pm.

POST: If you want your prescription to be posted back to you, remember to enclose a stamped addressed envelope, as we will be unable to do this otherwise.
