 When
ordering a repeat prescription please give the following
information:
Name;
Date
of Birth;
Medicines required;
Where you wish
to collect your prescription, e.g. Peak Pharmacy
(next door), Lloyds Pharmacy (Birley Moor Road
or Crystal Peaks), or at the surgery.

PHONE: 0845 1221881 and press option 2 - AT ANY TIME, day or night.
ONLINE: to be directed to the SystemOne Online Prescriptions Service.
Please note this is a new link, which takes you directly into our clinical system and should be even easier to use!
For first time use please phone us to get your username and password. Please change your password to something you can easily remember , once you have logged in for the first time.
Please let us know if you have any problems with our new system.
CALL IN: Bring
your prescription request into the surgery at any
time between 8:30am and 6:00pm.
POST: If you want your prescription to be posted back to you, remember to enclose a stamped addressed envelope, as we will be unable to do this otherwise. |
